Pop Quiz For The Paramedics

What infarct impostor presents with global ST segment elevation, and pleuritic, non-exertional chest pain that worsens dramatically with deep breaths, lying supine or bending over, but lessens significantly when leaning slightly forward?

I'm betting I can find no less than twenty knuckle-dragging, poorly-educated medics who can come up with the correct answer faster than the highly-educated, master's degree nurse practitioner that seemed bent on killing a friend of mine this past week.
